Prenatal / Diabetes Worker
(ADI / CPNP Coordinator)
Location: Fox Lake Cree Nation Office – Bird, Mb.
Start Date: July 28, 2010 – PERMENANT POSITION
Salary: Depending of education/work related experience
Position Summary:
Aboriginal Diabetes Initiative (ADI):
- Become familiar with Manitoba First Nations Diabetes Strategy
- Become familiar with the Manitoba Diabetes Care Recommendations
- Facilitate educational sessions for community member on basic diabetes prevention and treatment topics including healthy eating, physical activity, prevention of complications secondary to diabetes, self blood glucose monitoring and self care
- Provide ‘hands on’ healthy cooking sessions for people with/or at risk of diabetes
- Work in partnership with the community store to promote and ensure ongoing availability of a ‘core selection’ of healthy foods
- Facilitate a partnership with the school to look at healthy foods and physical activity policies and healthy life style education
- Provide feedback and communication to the Manitoba First Nations Diabetes Committee on the implementation of the Manitoba First Nations Diabetes Strategy in the community
- Implement the prevention and promotion, care and treatment and life style support activities as indicated in the yearly ADI work plan
- Develop yearly work plan for diabetes in partnership with the leadership and community
- Partner with other health programs and external resources as required
Canadian Prenatal Nutrition Program (CPNP)
- Become familiar with the CPNP community work plan national guidelines for the CPNP program
- Become familiar with ‘Nutrition for a Healthy Pregnancy,’ ‘Guidelines for the Child Bearing Years’ ‘Guidelines for Healthy Term Infants’
- Facilitate nutrition education sessions for prenatal and postnatal women on a variety of topics including healthy eating during pregnancy, breastfeeding and age appropriate introduction of solid foods
- Provide ‘hands on’ healthy cooking sessions for prenatal and postnatal women
- Work in partnership with the community store to develop and implement a
- CPNP voucher program for women who are pregnant and beast feeding
- Implement and develop the yearly work plan for the CPNP program for the community
Overall:
- Must be willing to Participate in ongoing training opportunities
- Willing to work with leadership to promote and support healthy community policies
- Act as a diabetes and prenatal resource for community members
- Fulfill reporting requirements of both the ADI and CPNP programs
- Applicant should have a minimum of Grade 12 or equivalency
